If Your Heart or Breathing Stops

If your heart or breath stops  ....

  • Your doctor will try to restart it.
  • Sometimes people do not wish to be revived.
  • If this is what you want, you can write that in the Wyoming Advance Health Care Directive form.

Filling Out A CPR Form

If you don't want to be revived, there is something else you can do. 

You can fill out a "Cardiopulmonary Resuscitation" Form or a "CPR Form".

  • You and your doctor must sign and date the CPR Form.
  • You must send the signed CPR form and $20.00 to the address below.
  • They will send you an identification number and put your name a list of people making the same choice.
  • They will send a CPR bracelet to you.
  • As long as doctors have your form or can see your bracelet they will not revive you.

How do I get my CPR Bracelet?

  • Calling or writing this office is the only way you can find out about a CPR form and how to get your bracelet.
