If you don't want to be revived, there is something else you can do.
You can fill out a "Cardiopulmonary Resuscitation" Form or a "CPR Form".
You and your doctor must sign and date the CPR Form.
You must send the signed CPR form and $20.00 to the address below.
They will send you an identification number and put your name a list of people making the same choice.
They will send a CPR bracelet to you.
As long as doctors have your form or can see your bracelet they will not revive you.
